Investigation Report of AS 350 Helicopter Crash

 

An investigation commission team comprising of Captain Siddhartha Jung Gurung, Engineer Janak Thapa and Pramod Nepal led by Buddhi Sagar Lamicchane submitted their detailed report to the Minister of Culture, Tourism and Civil Aviation Kripasur Sherpa. The report pictures the course of ill-fated events that led the chopper’s main motor hit an electrical neutral line.

In a report, the investigative teams outlined the major key factors that led to the incident. Main contributing factors were the passenger distraction, lack of situational awareness, cumulative fatigue, personality traits, scarce wire avoidance training, and no obstacle markings and the commission also issued some aviation safety recommendations to Civil Aviation Authority of Nepal (CAAN), Nepal Electricity Authority and Mountain Air to avoid such incidents in near future.

On June 2, AS 350 chopper met with an accident in a mountain region of Yamuna Danda in Sindhupalchowk, killing all four on board.
